Job Summary: Plans and coordinates activities related to discharge planning. Develops and implements a plan of care designed to meet the patient’s post hospitalization needs while ensuring continuity of care. Responsible for the collaboration with medical staff, patients, family, and the Healthcare team by assessing, facilitating, planning, and advocating for health needs on an individual basis resulting in quality, cost-effective outcomes.

Job Description:
Essential Functions:
- Assists patients to maintain their sense of competence, identity, and autonomy. Acts as an advocate for the patient/family.
- Demonstrates assessment/collaboration with hospital personnel and community resources to facilitate appropriate patient disposition while complying with organization and regulatory guidelines.
- Effectively utilizes community and regional resources to facilitate appropriate referrals.
- Participates in planning and continued process improvements.
- Assists in early identification of patients needing discharge planning intervention.
- Facilitates planning for patient/family needs to ensure a smooth transition for the patient across the continuum of care.
- Uses knowledge of community and regional resources, including benefits, eligibility standards, and qualifying criteria in developing a discharge plan, as documented in patient chart. Uses age-specific standards to access services.
- Expedites decision-making process related to the discharge plan and/or counseling through collaboration.
- Keeps patient, family, and outside agencies informed of discharge planning activities and tentative/concrete discharge dates.
- Maintains documentation of progress and status of the discharge plan on patient chart on a daily basis.

Required:
Case Management:
Education - Completion of a nursing education program that is approved by a board of nursing or Bachelor's in Social Work.
Certifications - Registered Nurse (RN) - South Dakota Board of Nursing or Certified Social Worker (CSW) - South Dakota Department of Social Services.
Behavioral Health:
Education - Bachelor's degree in Human Services.
Preferred:
Experience - 3+ years of Discharge Experience.
Physical Requirements:
Light work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
